​At Citrus Midwifery, we believe in supporting physiologic birth while offering effective comfort tools that help mothers feel safe, empowered, and cared for. One of the most effective non-medication techniques we use for back labor relief is called 
sterile water injections.
This simple, evidence-based technique can significantly reduce back labor pain while allowing mothers to remain mobile, alert, and fully present in their birth experience.

What Is Back Labor?
Back labor is intense pain felt in the lower back during contractions. It affects about 1 in 3 women and may feel like:
• Constant lower back pressure
• Sharp or deep aching pain
• Pain that continues between contractions
• Difficulty finding relief with movement alone
Back labor is often associated with baby’s positioning and pelvic pressure.

What Are Sterile Water Injections?
Sterile water injections involve placing small amounts of sterile water just under the skin of the lower back using a very small needle.
These injections:
• Take less than one minute to administer
• Contain no medication
• Are safe for mother and baby
• Provide natural pain relief
Midwives have used sterile water injections safely for decades.

How Do Sterile Water Injections Work?
Sterile water injections activate the body’s natural pain-blocking system.
The brief stimulation from the injection signals the nervous system to release endorphins — the body’s natural pain-relieving hormones.
This helps interrupt the pain cycle and reduce back labor discomfort.
Most mothers experience relief within:
• 2–10 minutes
• Lasting 1–3 hours

When Do Midwives Use Sterile Water Injections?
We may recommend sterile water injections when a mother is experiencing:
• Severe back labor
• Persistent lower back pain
• Fatigue from prolonged discomfort
• Desire to avoid epidural or medications
• Limited relief from movement or hydrotherapy
They can be used in early labor, active labor, or repeated if needed.
